Annual Fly Periodic Health Assessment/Flight Physical
The objective of the PHA/MHA is to determine a service member’s fitness for duty in accordance with service regulations and policies. The Annual Fly Periodic Health Assessment Questionnaire MUST be completed prior to scheduling an appointment.
-
Complete PHAQ on the ASIMS website. (NOTE: Link underline words to https://asimsimr.health.mil/imr/myimr.aspx)
-
Once your PHAQ is completed, you will be given instructions to schedule the Mental Health Assessment portion of your PHA on the ASIMS website. The PHA/MHA is NOT conducted by the 78 MDG Mental Health Clinic. The PHA/MHA is a virtual, by-phone appointment that takes less than 10 minutes to complete. The MHA provider will attempt to call you twice within your scheduled appointment time. If you don't answer, you will be marked as a no-show.
Flyers must have their MHA appointment completed as part of their Annual Fly PHA appointment.
